If a student wants medication for anything, he or she must have a student health form on file in the clinic before the nurse is legally able to give medication. Also, due to state law, the nurse must SPEAK with a parent before medication is administered. If you didn't receive a form from your homeroom teacher, please visit the nurse's office to pick up one or print one here.
